Legal Agreement

I understand that VBS, as any childhood activity, involves certain risks for physical injury. I understand that Bel Forest Baptist Church does not insure participants in this activity, that any coverage would be through personal insurance, and Bel Forest Baptist Church has no responsibility or liability for injury resulting from this activity.

I, the undersigned parent/guardian, do hereby authorize adult volunteers of Bel Forest Baptist Church as agent(s) for the undersigned, to consent to any emergency medical or surgical care deemed advisable by an accredited physician or surgeon in an approved emergency clinic or hospital.

I further release from any liability Bel Forest Baptist Church and any of its ministries or leaders in the event of an accident enroute, during and returning from the above mentioned event. This agreement does not apply to claims for intentional misconduct or gross negligence.
